Obtaining the harmonica is straightforward. In fact, you might already have it without realizing. Most players come across it while searching for the music player provided by the Musician prepper. However, despite being the obvious candidate, he doesn't hand over the harmonica.
Actually, no one gives it to you directly. It simply appears in Sam's inventory. The harmonica becomes available in Sam's headwear selection after completing Main Order No. 8 and arriving in Australia. Dollman may mention this during a rest period after some walking, but due to the early Australian areas being packed with BTs, many players miss the notification.

Playing the harmonica is easy and functions just like in the original game. All you need to do is open Sam's headwear menu during gameplay and equip it. It can be used together with any other headwear items without conflict.

So, what does the harmonica actually accomplish? In the previous title, it was valuable because it offered music in a game where soundtracks were only present during key story moments. Since Death Stranding 2 includes a music player for creating custom playlists, the harmonica feels less essential now.
To an extent, that’s true. Its original purpose is mostly unnecessary in this sequel, except during longer stretches spent outside the reach of the Chiral Network. Still, it retains one important function that the music player lacks.
